MS Conference Room
The District Service Center processes all requests for the use of District facilities and the posting of banners on District property, ensures that all user groups are properly insured and meet all of the District policy requirements for use of school facilities, and coordinates with the Maintenance and Operations Department to ensure proper maintenance of the facilities.
Any individual, group, school or non-school organization that would like to use any of the Middle School meeting rooms must complete the MS Room Application for Reservation online form.
Any approval for use of District property is for the requestor only; time may not be sublet to another organization.
In requesting the usage of any of the Middle School meeting rooms, you must complete all the appropriate information on the MS Room Application for Reservation online form. This information is vital in meeting your needs and making sure your event is carried out successfully. Failure to fill-out information or check any appropriate boxes will create a delay in possible approval of your request. In addition, all requests are reviewed on a first come first serve basis. In reviewing your information, please make sure you have completed the following information:
- Name, email, address and a working telephone number in case you need to be reached.
- Organizational Status (profit/non-profit) and whether a fee will be charged or not.
- Number of people expected to attend.
- Which part of the facility you are requesting.
- If a fee is being charged to enter the event.
- If you are having any items sold as a concession – we need to know.
- Do you need any specific equipment (we may not be able to provide everything)?
- Date and time of event. (This will not include set up or clean up time by our custodial staff.)
- Finally make sure you have signed all sections of the request form.
Any information not provided or requested may result in additional charges and denied permission for such requests on the day of your event. Once the MS Room Application for Reservation online form is submitted you will receive an email, within seven day, from the Middle School Business Office.
Upon receiving the completed form from the individual, group, school or non-school organization and sponsoring group, dates will be reviewed to determine availability of the room request, and if applicable, room usage fees will be determined. Once approved or denied, the individual, group, school or non-school organization and sponsoring group will be notified of the Middle School Business Office’s decision.
