Physical/Dental Exams & Health Screenings
To support the health and well-being of every student, Pennsylvania law requires certain physical and dental examinations, as well as routine health screenings throughout a child’s school career. These services help identify potential health concerns early and allow families to seek appropriate follow-up care when needed.
Unless otherwise noted, required physical and dental examinations should be completed by your child’s healthcare provider and the completed forms returned to the school nurse.
Physical Examinations
Physical examinations are required:
- Upon original entry to school (Kindergarten or first grade)
- Grade 6
- Grade 11
Physical examination forms should be completed by your child’s family physician or healthcare provider and returned to the school nurse.
Dental Examinations
Dental examinations are required:
- Upon original entry to school (Kindergarten or first grade)
- Grade 3
- Grade 7
Dental examination forms should be completed by your child’s family dentist and returned to the school nurse.
Annual Health Screenings
The following health screenings are conducted by the school nurse in accordance with Pennsylvania requirements:
- Height & Weight: All students (K–12)
- Vision Screening: All students (K–12)
- Hearing Screening: Grades K, 1, 2, 3, 7 and 11
- Scoliosis Screening: Grades 6 and 7
