Transcript Release Form

By filling-out the Transcript Release Form, the student of the application is authorizing Loyalsock Township High School to send their transcript, which contains a record of all high school courses and their class rank to any post-secondary institution(s) upon that student’s request. Students/families must complete and sign this form before your school counselor or Mrs. LaPoint can mail their transcript to post-secondary institutions.

Please note that starting in 2016, it became the responsibility of the student to send their SAT and/or ACT scores to colleges and universities. Students can do this by signing into their SAT and/or ACT accounts and sending scores directly to the post-secondary institutions through the testing site’s website.

All LTHS graduates should contact Mrs. Carrie LaPoint, counseling secretary, in order to obtain a copy of their transcript. Please contact Mrs. LaPoint at clapoint@loyalsocklancers.org or 570-326-3581 ext. 1301.
